As a Social Service Designee, you will play a critical role in continuing the healing mission of Deaconess.

Job Overview

Assists patients and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care by performing the following duties.Works in close collaboration with members of the health care team to provide services and optimize patient functioning in the most efficient and effective manner.  Performs comprehensive evaluation including social, emotional, cultural, mental status, environmental and financial circumstances in conjunction with interdisciplinary assessment, and recommends a plan of intervention based on mutually established patient goals. 
 
The Social Services Designee must be able to demonstrate knowledge of growth and development to coordinate patient care appropriate to the age of patients served on his/her assigned clinical area.

Assessment  
Assess the needs of the patient and family, when appropriate and arrange, coordinate, monitor, evaluate, and advocate for multiple services to meet the specific patients’ complex needs.
Assessment is based on relevant physical, emotional, developmental, social, cultural, spiritual, and sexual data to identify needs and determine real and/ or potential patient risks.  Assessments indicate knowledge and understanding of the medical plan of care including but not limited to treatment goals, orders, pertinent diagnostics and laboratory results, ancillary care plans, interventions, and discharge plans.

Intervention  
Develops and implements routine and complex discharge planning including nursing home, rehabilitation, long term acute placements and community resource referrals in a timely manner to improve flow and reduce length of stay without compromising safety and quality of the discharge.
Facilitates transfers to acute care facilities and psychiatric hospitals.
Ensures that each patient receives appropriate assistance by providing accurate and complete information about the extent and nature of available services and by helping the patient/family decides which services will best meet his/her needs.

Certifications/Licenses/Education

Graduate of a Masters of Social Work Degree required
Experience in health care, crisis intervention, counseling and/or social agency preferred.    No licensure or certification required.
