The Opportunity

Deaconess Health System in Evansville, Indiana seeks a high caliber Manager of Case Management The health system encompasses 10 hospitals, the Deaconess Clinic, a fully integrated multispecialty group, as well as multiple care sites and partnerships with other community health care providers.  With 7,500+ employees, total of 778 licensed beds and $1 billion in operating revenues, Deaconess has become the premier provider of health care services to a 26-county, tristate area in southwestern Indiana, western Kentucky and southeastern Illinois.    

As a successful Manager of Case Management, you will promote the advancement of coordinated, cost-effective, quality patient care and services within two acute care hospitals in partnership with the health system care coordination strategy. Provide oversight and direction of the care coordination department functions for Deaconess Hospital, Inc. This will involve working closely with Department Managers across nursing units to integrate unit-based case management and social work functions with the unit care delivery system benefiting care coordination. Collaborates with nurse leaders at all levels to promote evidence-based practice and standards of care across nursing units. In addition, collaborate with other department managers and directors, health care providers; information services leaders to meet Deaconess Hospital, Inc. annual goals and strategic priorities.

Responsibilities

STRATEGIC LEADERSHIP. Communicates the organization's vision and values to be carried out in the day-to-day activities throughout Deaconess.

URSING PRACTICE & PERFORMANCE. Serves as a patient advocate seeking to optimize the patient experience and ensure timely and appropriate discharge/transfer planning.

QUALITY, SAFETY & COMPLIANCE. Creates an environment for organizational performance that supports quality and safety improvements and adheres to applicable laws and compliance regulations

HUMAN RESOURCE MANAGEMENT & WORKFORCE ENGAGEMENT. Engages, manages, and develops staff to utilize their full potential in alignment with mission, vision, and strategic initiatives.

FISCAL AND RESOURCE MANAGEMENT. Responsible for effective and efficient fiscal management of department operations to ensure proper utilization of organizational and financial resources.

Requirements

Baccalaureate degree in nursing is required. Master’s degree in nursing or management/business required. Maintain competency and continuing education in area of specialization and in professional practice required. Five years of hospital clinical nursing experience recommended. Three years of management experience required. Experience with case management supervisory functions in a large health system preferred. Demonstrated project management skills using six sigma methodology required. Knowledge of EPIC electronic health record preferred.  Must have current RN license in the State of IN.
