SUMMARY OF THE JOB
  • Provides comprehensive nursing care to patients. Is responsible for the clinical supervision of Aides, Licensed Practical Nurses, and student nurses in the management of the patient's plan of care.
 
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
  • Job Duties include the following, other duties may be assigned:
      • Provides holistic treatment, preventive, and rehabilitative services and provides the oversight and management of services to individuals and families within their homes. These services include but are not limited to:
        • a. Establishment of health care goals and objectives to improve patient care outcome.
        • b. Recommendations for solutions for patient's needs including appropriate referrals to other disciplines or community resources.
        • c. Teaching and supervision of patients and support system in their nursing care.
        • d. Recognizing physical and emotional illnesses and/or disabilities and assisting patients in securing appropriate referrals.
        • e. Administering medications and treatments as prescribed by the patient's physician.
      • Conforms to established Agency expectations of productivity.
      • Identifies and reports significant conditions and symptoms to physicians and other health care members; updates physician's orders according to agency policy.
      • Documents nursing care and other significant information in a timely fashion. Maintains a clinical record that meets the requirements of policy and procedure and accepted standard of practice for each patient receiving care.
      • Is responsible for coordinating patient care, promoting continuity of patient care, and communicating with other departments and on-call supervisor as appropriate.
      • Improvises and manipulates the patient's environment so that skilled nursing care can be adequately provided.
      • Demonstrates awareness and support of the Agency’s outcome objectives.
      • Participates in on-call, holiday coverage, weekend rotation as necessary to provide 24/7 nurse availability.
      • Participates in Agency and community health activities as requested.
      • Participates in case conferences as part of the interdisciplinary team.
      • Assists in the orientation of new personnel and/or student nurses.
      • Supervises the LPN and/or Aides by communicating and updating the plan of care.
      • Attends meetings and/or workshops as requested.
      • As assigned, receives referral information and initiates admission of the patient to Deaconess VNA Plus services.
      • Accepts responsibility under adverse working conditions, within the limits of safety.
      • Respects personal and medical information in the strictest confidence.
      • Driving is required for visiting staff
      • Performs all other duties as may be required.

 JOB REQUIREMENTS & QUALIFICATIONS

  • Education and Experience
    • Must have graduated from an accredited school of nursing. Bachelor’s degree preferred. Community health, critical care and/or oncology experience preferred.
  • Certificates, Licenses, Registrations
    • Based on geographic assignments, must have an RN in  Indiana, Illinois, and/or Kentucky. Six (6) months nursing experience preferred. Valid Driver's license required for visiting staff
  • Language Skills
    • Ability to read, analyze, and interpret professional references, technical procedures/protocols, or governmental regulations. Ability to write reports and other forms of correspondence. Ability to effectively present verbal and/or written information, and respond to questions, to all customers. Ability to write/communicate clinical information to appropriate individuals.
  • Mathematical Skills
    • Ability to apply mathematical concepts such as addition, subtraction, multiplication, division, fractions, percentages and proportions, to calculate dosages, amounts and volumes.
  • Reasoning Ability
    • Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
  • Computer Skills
    • This job requires computer and systems competency in the following areas: enters data; retrieves data; assembles and organizes data, uses and analyzes data, integrates diverse sources of data. The ability to learn and use the provided electronic medical record system within six months of attaining nursing position.

Independently moves or lifts up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.

      • While performing the duties of this job, the employee may be exposed to varying temperatures outdoors and within residence. Exposed to a variety of environments in patient homes, including but not limited to: animals, smells, pharmaceutical agents, chemicals, dust, lint, and dander.  Visiting employees will be required to operate a vehicle and may experience driving hazards.
